Diagnostic Value of Bone Marrow Culture in Typhoid Patient

Abstract
Recovery rate of Sal. typhi from bone marrow culture was compared with the rates from blood, stool and urine culture in total of 59 patients of typhoid fever who had been hospitalized to the Dept. of Internal Medicine in National Medical Center during 5 months from April to August 1976.
1) High rate of positive growth was found from bone marrow in 44 (74.6%) cases while blood and stool were positive in only 30 (50.8%) and 11 (25.6%) cases, respectively.
2) The positive rate of ordinary site of bacterial culture from blood, stool and urine was 57.6% (34 cases) only, but additional bone marrow culture, the total positive rate was 81.4% (48 cases), and it was higher in rate than 79.7% of serological positive result.
3) Single positive growth from bone marrow culture alone was 14 (23.8%) patients and moreover Widal test was negative in 5 (8.5%) of them; in contrast, single positive growth from blood alone was. 2 (3.4%) cases but Widal test was positive simultaneously. This result reflects that without bacterial culture from bone marrow, the bacteriological diagnosis might be missed in 23.8%, and moreover diagnosis of the disease might be missed in 5 (8.5%) of them with negative Widal test.
4) The positive growth rate of Sal. typhi from bone marrow was highly efficient throughout any weeks of the infection period and also inspite of previous antibiotic medication.
5) These results may propose that bone marrow culture is very effective diagnostic tool for typhoid fever and also bone marrow culture must be performed routinely in clinically suspected cases of typhoid fever.
